Disclosure Text on the Protection of Personal Data

As Ozel Dena Agiz ve Dis Sagligi Poliklinigi ("Dena Clinic"), acting as data controller under Law No. 6698 on the Protection of Personal Data ("KVKK"), we would like to inform you about the processing of the personal data you share with us through our website.

Personal data processed

When you fill in the contact/appointment form, your first and last name, phone number, email address (if provided), the service/topic you request, your preferred date/time, the information you share in your message, and technically your IP address may be processed.

Purpose of processing and legal basis

Your personal data is processed for the purpose of assessing your appointment and contact request, getting in touch with you, and informing you about our clinic's services, based on the legal grounds set out in Article 5 of the KVKK - namely "explicit consent" and/or "being directly related to the establishment or performance of a contract".

Transfer of data

If you choose to send the form via WhatsApp, the message content is transferred to WhatsApp as part of your own action; this transfer is subject to WhatsApp's own privacy policy. Your personal data is not shared with third parties except where legally required.

Retention period

Your personal data is retained for as long as required by the purpose of processing, subject to the statutory limitation periods set out in applicable legislation; at the end of this period it is deleted, destroyed or anonymised.

Your rights as a data subject

Under Article 11 of the KVKK, you have the right to: learn whether your personal data is being processed; request information about it if it is; learn the purpose of processing and whether it is used in accordance with that purpose; know the third parties to whom it is transferred domestically or abroad; request correction if it has been processed incompletely or incorrectly; request its deletion or destruction within the conditions set out in the KVKK; request that any correction or deletion be notified to third parties to whom the data was transferred; object to a result that is to your detriment arising solely from analysis of the processed data through automated systems; and request compensation for any damage you suffer due to unlawful processing.
